We’ve compiled these Tennessee divorce resources in order to help you if you are considering or about to proceed with a divorce.

Short Form of Statistical Information
The Short Form of Statistical Information is used by our office to gather basic information about the parties involved in a Tennessee or Mississippi divorce, covering all aspects from full name and social security number to marriage and separation dates.  When filling out this form it is crucial to answer each question fully, honestly, and in detail.

Divorce Checklist
The Divorce Checklist was developed by our office to help you gather your thoughts before moving forward with the divorce process.  It is an especially helpful document in a contested divorce matter in its ability to help you organize your questions and potential issues before speaking with an attorney.

Marital Balance Sheet
Our Marital Balance Sheet is used to determine the division of marital assets and debts.  It gives you space to write out such items as real estate, cars, loans, 401k accounts, credit card debts, and college loan debts.  When you fill this document out it is essential to be as detailed as possible in indicating which party will assume responsibility for each debt or asset, including the last four digits of account numbers, the make, model, and year of cars, specific addresses of real estate, etc.

Notice Regarding Insurance
The Notice Regarding Insurance is a required document for every Tennessee divorce proceeding, whether or not one party provides health insurance for the other.  The key information needed for this document includes: the name of the insurance company involved (if there is one), the policy ID number, and the Plan Administrator at the providing party’s Human Resources department for continuation of coverage through COBRA.  Your attorney can alter the document to indicate that neither party provides health insurance coverage for the other should that be the case, but each divorce proceeding should have this document filed with the Courts.

Permanent Parenting Plan
The Permanent Parenting Plan will ultimately serve as the guideline for all issues regarding the children involved in a divorce, covering such points as visitation schedules, holidays, insurance, and child support, among others.  This document needs to be thoughtfully filled out by both parents (when possible) to create a plan that is in the best interest of the children.  Each and every blank needs to be filled, to ensure that a thorough and comprehensive plan has been created.  It is important to think to your future rather than your current situation – for example, if your child is three years old, make sure to anticipate all of the scheduling rigors that school will bring, and create a visitation schedule accordingly.  Please refer to our Permanent Parenting Plan Instructions for step-by-step help in filling out your Parenting Plan.

Child Support Data Checklist
We have created this checklist to assist our clients in compiling all of the information necessary to complete the next document, the Income Shares Worksheet.  It is best to have explicit documentation of each of these pieces of information (i.e. pay stubs or letters from the Human Resources departments of the parties’ employers) whenever possible, especially in the case of a contested divorce here in Memphis.

Income Shares Worksheet
The Income Shares Worksheet is a document created to assist divorcing parents in calculating child support in accordance with state guidelines.  It requires the input of data collected from the Child Support Data Checklist (shown above) to automatically calculate the child support payment amounts required.  Your attorney can best assist you in completing this complicated document.

Parenting Class Providers
Divorcing parents of Shelby County are required to take a Parenting Class to finalize their divorce.  It is a one-time class, and the certificates indicating completion are filed with the Courts as part of the divorce documentation.  We have provided here a list of places to take this class, although we must mention that this list is always evolving.
